330 | |a Objective: To assess whether the optimization of cerebral oxygenation based on regional cerebral oxygen saturation (rSO2) monitoring reduces the occurrence cerebral ischemic lesions. Design, Setting, and Participants: This open-label, randomized, double-blinded, controlled trial, conducted at 19 vascular surgical units in France, recruited adults admitted for scheduled carotid endarterectomy. Patients were enrolled between May 3, 2011, and April 27, 2016. Interventions: Optimization of cerebral oxygenation based on rSO2 monitoring using near-infrared spectroscopy group (NIRS, n = 437) or standard group (n = 442). A decrease in rSO2 of 15% or more involved a step by step intervention of an increase in oxygenotherapy, an increase in capnia, lifting the legs, volume expansion and use of vasopressor support. Main outcomes and measures: The primary endpoint was the number of new cerebral ischemic lesions detected using magnetic resonance imaging pre- and postoperatively. Secondary endpoints included neurological and cardiovascular postoperative complications, in-hospital length of stay, quality of life, and mortality at day 120 after surgery. Results: 879 patients were randomized. There was no statistically significant difference between groups for the mean number of new cerebral ischemic lesions per patient up to 3 days after surgery: 0.35 (+/- 1.05) in the standard group vs. 0.58 (+/- 2.83), in the NIRS group; mean difference, 0.23 [95% CI, -0.06 to 0.52]; estimate, 0.22 [95% CI, -0.06 to 0.50]. Postoperative neurological disorders up to day 120 after surgery were not different between the groups. The mortality rate at day 120 was not different between the groups. Conception : essai randomisé, contrôlé, en double aveugle, mené dans 19 services de chirurgie vasculaire en France. Population d'adultes programmés pour une endartériectomie carotidienne recrutés entre le 3 mai 2011 et le 27 avril 2016. Interventions : Optimisation de l'oxygénation cérébrale basée sur la rSO2 (NIRS, n = 437) ou une prise en charge standard (n = 442). 15 % de diminution par rapport à la rSO2 de base, impliquait les interventions séquentielles : augmentation de l'oxygénothérapie, augmentation de la capnie, épreuve de levée de jambe, expansion volémique et enfin un soutien vasopresseur. Critères de jugement : Le critère de jugement principal était le nombre moyen de nouvelles lésions ischémiques cérébrales détectées par IRM avant (J-1) et après (J+3) l'opération par patient. Les critères de jugement secondaires comprenaient les complications postopératoires neurologiques et cardiovasculaires, la durée du séjour à l'hôpital, la qualité de vie et la mortalité au 120e jour. Résultats : Il n'y a pas de différence statistiquement significative entre les groupes sur le critère de jugement principal : 0,35 (+/- 1,05) dans le groupe standard contre 0,58 (+/- 2,83) dans le groupe NIRS. Les troubles neurologiques postopératoires jusqu'au 120e jour après l'opération n'étaient pas différents entre les groupes. Le taux de mortalité à 120 jours n'était pas différent entre les groupes.
