Au stade initial, elle pourrait compenser, au moins un temps, les conséquences de la dénervation dopaminergique. A un stade plus tardif, elle pourrait participer à la genèse des dyskinésies. Cette thèse se propose de mettre en évidence ces mécanismes adaptatifs en évaluant l excitabilité corticale motrice par stimulation magnétique transcrânienne à deux stades évolutifs de la maladie. 1) A un stade précoce, avec pour hypothèse l existence de mécanismes de compensation au niveau de l'hémisphère cérébral asymptomatique (HA). A ce stade, l asymétrie motrice ne se reflète pas dans l excitabilité. L inhibition précoce tend à être moindre dans HA, moins sensible au traitement dopaminergique, que chez les sujets témoins tandis que l excitabilité de l hémisphère symptomatique (HS) se comporte normalement, suggérant de possibles mécanismes de compensation. La facilitation tardive pourrait être précocement altérée avant même l apparition des signes moteurs, les différents circuits testés pouvant avoir une sensibilité différente selon le niveau de dénervation. 2) A un stade évolué, avec pour hypothèse l existence d une plasticité défavorable chez les patients dyskinétiques. La lévodopa, en dépit d un bénéfice moteur équivalent chez les patients dyskinétiques ou non, n'aurait pas chez les patients dyskinétiques d'effet sur l excitabilité de certains circuits inhibiteurs qui ne pourraient donc pas jouer leur rôle de manière efficace, libérant des programmes moteurs parasites auparavant inhibés. | ||
330 | |a Plasticity of the central nervous system could have a positive or negative effect at different stages of Parkinson Disease. On one hand, compensatory mechanisms (CM) might counterbalance the consequences resulting from brain dopamine (DA) deficiency at early stage, and on the other hand, they could be involved in the genesis of levodopa-induced dyskinesia at advanced stage. We have studied these CM evaluating motor cortical excitability through transcranial magnetic stimulation at these two stages. Our hypothesis was that CM might exist at early stage in the hemisphere contralateral to the clinically unaffected side of the body (UH) whereas a DA deficiency is already present and could be associated with changes in the motor cortex excitability. We found that the motor asymmetry was not sustained by an asymmetric excitability. The short intracortical inhibition (SICI) tended to be lower in the UH than in the hemisphere of healthy subjects and was less sensitive to levodopa in the UH than in the affected hemisphere (AH). However SICI in the AH was strictly comparable to healthy subjects suggesting the presence of some CM in the UH. The late facilitation seemed to be affected earlier, preceding the development of motor signs. At the advanced stage, according to our hypothesis, an aberrant plasticity was evidenced in the dyskinetic patients.Although levodopa treatment improved motor symptoms at the same extent in both dyskinetic and non dyskinetic patients, the excitability of the inhibitory pathways appeared to be lower in the dyskinetic patient. This may explain the easy release of non desired movements, i.e. the dyskinesia, when levodopa acts in these patients. | ||
